Dear Sandberg Religious School Families,
We continue this year with our online system for registration. This letter will explain to you the procedure for signing your child/ren up for the 2012-2013 school year.
1. The registration form was created to be used with a Google Docs Spreadsheet and is on a secure site.
2. Similar to some online “surveys,” the entire form must be filled out in one sitting as there is no way to “save” the data. Here is what you will need to have on hand in order to complete the form:
a. Physician—address, phone number
b. Dentist—address, phone number
c. Emergency contact/s—phone numbers
d. Allergy information
e. Health Insurance company—address, group/individual policy number, phone number
f. Secular school contacts for accommodations/special needs if your child is receiving servicesg.
g. Names of adults authorized to pick up your child from school
h. Families/students in your carpools

3. Below is the fee schedule for school tuition for 2012-2013, unchanged from last year. Our Bookkeeper will bill you at the following rates:
Shorashim (K/1st Grade) and Sundays, 9:00 a.m.-Noon $725
Kitah Bet (Second Grade)
Gimel through Zayin Sundays, 9:00 a.m.-Noon and
(3rd-7th Grade) Tuesdays, 3:45-6:00 p.m. $950
Each additional child (in Gimel through Zayin) $825
